I am not aware of any other conventions. Just verse that has the name YHVH, the three letters of the name anywhere in the verse outside the name YHVH, and that they should be in order.

Yes. I don't even bother trying to keep track of all the places the paper was copied to. Someone even translated it to French without giving me credit.

I'm uncertain. IIRC, there are a few verses where Kircher mis-quotes the original Hebrew.

Also, there's a site with recordings in mp3 format of a person reading the bible by chapter http://www.mechon-mamre.org/i/t/tmp3prq.htm. Note the reader announces the beginning of every parasha aloud.

Is the AL or YH endings of the angel names determined by a rule of some sort?

My preference is for the letters of the name to appear in the proper order (reading right to left). I note in the example of Kircher's 'backwards' version of VVL :

Psalm 88:13 (well, 88:14 in the Tanakh)

וַאֲנִי, אֵלֶיךָ יְהוָה שִׁוַּעְתִּי; וּבַבֹּקֶר, תְּפִלָּתִי תְקַדְּמֶךָּ.

For Kircher, this is where he sees the name:

V-ANI ALIK YHVH ShVOTI V-B-BQR TPLTI T-QDM-K

The name is also there in the 'correct' order:
V-ANI ALIK YHVH ShVOTI V-B-BQR TPLTI T-QDM-K

The three words with the letters of the name then being:
שִׁוַּעְתִּי; וּבַבֹּקֶר, תְּפִלָּתִי

(I ask, in the morning, prayer)

Asked the same question myself, and haven't found an answer yet.

I've never seen those endings in Jewish sources. On the other hand, I wouldn't be surprised to find them one of those days.
